HEALTH CERTIFICATE REQUEST

Health Certificates: $14.20 if Coggins & CORE EQ & Flu-Rhino Vaccines have been administered by Huck Equine Clinic. 
 

A health certificate is valid for 30 days after signing to one specific location. The health certificate may include any horse(s) that are owned by you. If a horse owned by another individual is going, they will have to have a separate health certificate.
