HEALTH CERTIFICATE REQUEST

$13.00 per certificate if the horses receive the CORE (Tetanus, Rabies, West-Nile, Eastern and Western Encephalitis) Vaccine through us.
$30 per certificate if no CORE Vaccination.

A health certificate is valid for 30 days after signing to one specific location. The health certificate may include any horse/s that are owned by you. If a horse owned by another individual is going, they will have to have a separate health certificate.
